Whether you have anxiety about taking new medications, need help monitoring your symptoms, or just want someone to support you on your heart failure journey, we’re here to help. With the ultimate goal of helping you live a happier and healthier life, we partner with you to prevent readmission to the hospital and remain in your home.
How can you help me to take care of myself?
We help you to follow the discharge instructions given to you when you were released from the hospital. By following these instructions and changing your diet, we help you better care for yourself and prevent readmission to the hospital.
How do you help me follow my discharge instructions?
Through coaching, education, and support we help you:
Take medications as prescribed by your doctor.
Get to your doctor’s appointment.
Plan heart-healthy meals.
How long can you help me?
This depends on your unique wants and needs. We can help you for a week, a month, or longer!
